At Cañon Professional Wellness Clinic, we understand the importance of having a complete medical history for accurate diagnosis and treatment. If you need to transfer your medical records to or from our clinic, please follow the steps below:

• Request a Release Form
  Contact our front office to obtain a Medical Records Release Form. This form gives us the authorization to either release your records to another healthcare provider or obtain them from a different provider.
  
  
• Complete the Form
  Fill out the form with all the necessary details, including your full name, date of birth, and contact information. Also, provide the name and address of the clinic you're transferring records to/from and specific details about the records you want to be transferred.
  
  
• Submit the Form
  Return the completed form to our clinic. You can drop it off in person, fax it, or mail it to our Cañon City address.
  
  
• Processing Time
  Please allow up to 10 business days for the transfer process to be completed. If you're in a rush, let us know, and we'll do our best to expedite the process.
  
  
• Fees
  There might be a fee associated with the transfer of medical records, especially if you request printed copies. Electronic transfers are often free. Please check with our front office for details.
  
  
• Privacy
  Your medical information will be handled with the utmost care and confidentiality. We adhere to all HIPAA guidelines to ensure your privacy is protected.
